Clinic Updates

**CLOSURE FOR NY HOLIDAY 12/31/25-01/02/26. Provider and staff will return on 01/05/26.

**Effective immediately all NO SHOW appointments and Late Cancellation within 24 hours of scheduled time will have a $50.00 fee placed on accounts. This charge is NOT run through insurance.  This fee will need to be paid prior to any rescheduling of appointment.

**All cumbersome paperwork such as Disability, FMLA, LOA, Appeals, etc will have a $25.00 fee upon completion. This charge does NOT run through insurance. This is patient responsibility. 

**New Patient appointments will require a $25.00 deposit. This amount will be applied to visit once seen If visit is missed /no show the fee will be charged. 

**If you will have new insurance for 2026, please make sure to contact the office with this updated information PRIOR to your appointment.
