We accept insurance as well as self pay option.

We accept nearly all major insurance plans including:

  • Medicare, Medicare Supplements, and Medicare Replacements 

  • Traditional Medicaid as well as Amerigroup, Ambetter & Superior Health

  • Cigna Open Access only

Additional Fees:

There is an additional $25 dollar fee for the completion of forms for use outside of the patient's medical record, including:

  • Day Care

  • Pre-Employment

  • Employment

  • Life Insurance.

  • Family Medical Leave

Failure to cancel an appointment will be considered a NO SHOW and the charge is $25.00 for MON-THUR and $50.00 for FRI -SAT appointments!
